Life-Grounding CanMEDS: Reorienting Medical Competence within Canada’s Unfinished Synthesis of Peace, Health, and the Civil Commons

CanMEDS offers an influential answer to the question of what physicians should be able to do. It is less explicit about a prior normative question: what should those capacities ultimately serve? This paper argues that a life-ground lens can provide an orienting and corrigible answer without becoming an eighth CanMEDS role, subordinating rights or Indigenous jurisdiction, or weakening Medical Expert. The argument is developed through a critical-integrative reading of six Canadian strands: peacebuilding, Medicare, the Lalonde and Ottawa Charter health-promotion tradition, primary health care, CanMEDS, and Life-Value Onto-Axiology and the civil commons. Across these strands, a recurrent pattern appears. Public responsibility is normatively enlarged from isolated events and individuals toward relationships, structures, communities, ecosystems, and future generations, while operational systems of jurisdiction, finance, incentives, metrics, and assessment remain narrower. This purpose-implementation gap is termed operational contraction; under sustained pressure it can produce institutional inversion, in which means such as throughput, organizational survival, fiscal targets, or market value displace the life-serving ends institutions were created to support. Life-grounding makes that displacement visible by asking whether policies, institutions, and professional practices protect, restore, or enlarge life-capacities under conditions of rights, dignity, evidence, nonviolence, ecological integrity, legitimate authority, and public correction. Life-coherence is therefore defined not as stability or uniformity, but as the sustained capacity for materially grounded, relationally viable, dignity-preserving, nonviolent coexistence under changing conditions. CanMEDS is positioned as a strategic hinge because its seven roles connect clinical knowledge to communication, collaboration, leadership, advocacy, scholarship, and professionalism. Yet professional formation cannot compensate for life-disabling institutions. The paper therefore proposes a broader Canadian programme linking a Life-Ground CanMEDS Implementation Lab to community-governed primary health care, Health-Peace and Life-Impact Assessment, a Life-Coherence Dashboard, civil-commons investment, and protected institutional capacity for learning and moral agency. The July 2026 approval and October 2026 launch of CanMEDS 2026 create a timely institutional opening for testing this synthesis rather than presuming it.

THE DYNAMIC ARCHITECTURE OF VIABILITY: From Distinction, Energy, Constraint, and Absence to Sentience, Relation, and Life-Coherent Worlds

This monograph integrates a sequence of papers on distinction, autopoiesis, viability, emotional sentience, qualia, relationality, exceptional mathematics, love and life-value into one dynamic architecture. Its central proposal is that living reality is best approached neither as a hierarchy of substances nor as a list of disconnected explanatory levels, but as a historically recursive circulation. Energy gradients make work possible; constraints channel work; self-producing closure establishes a beneficiary for whom outcomes matter; perception detects viability-relevant differences; affect compresses their significance into valence; regulation selects and enacts responses; action alters the organism–world field; memory and learning sediment the history of coupling; and generative margin protects variation from which new options can arise. The loop is simultaneously material, experiential and relational, although these descriptions are not interchangeable.

The synthesis is organized by a triune-aspect process monism. A single relational process is accessible through a first-person window of feeling, a second-person window of participation and mutual address, and a third-person window of publicly describable organization. This is advanced as a metaphysical orientation, not an empirical result. It avoids multiplying substances while refusing to reduce lived presence or interpersonal recognition to third-person descriptions. It remains neutral between the claim that all reality is experiential and the more conservative claim that phenomenal experience occurs only when living regulation reaches a particular organization.

At the functional level, the framework proposes seven recurrent viability roles: constraint (C), margin (M), state (X), disturbance (D), perception (P), regulation (R) and options (O). Their seven balanced triads — CMX, XDR, XPO, MOR, CDO, CPR and MDP — form a Fano-plane incidence pattern. That incidence structure is exact once the mapping is stipulated, but the claim that these seven roles form a minimal cross-scale grammar is a bridge hypothesis. The Fano plane, octonions, Spin(8) triality, the exceptional Jordan algebra, Freudenthal systems and exceptional Lie groups are therefore treated with a double discipline: their mathematical relations are exact; their proposed relevance to biological, experiential and social organization is conjectural and must earn empirical standing.

Deacon’s account of constraint, absence and teleodynamics clarifies how a process can be organized around what is not yet the case. Friston’s free-energy formulations clarify how adaptive systems can maintain themselves through inference and action under uncertainty. Maturana and Varela clarify operational closure, structural coupling, cognition and the observer-dependent constitution of explanatory domains. Peil Kauffman, Panksepp, Damasio and Solms clarify the biological and neural organization of valence and feeling. Wilber’s AQAL and Wilber–Combs frameworks help prevent category errors by locating interior and exterior, individual and collective, state and developmental stage. McMurtry’s life-value ontology supplies the normative test: an organization is life-coherent insofar as it protects, restores or enlarges life-capacity without transferring disabling costs to other lives or the future conditions of life.

The resulting framework is intentionally stronger than a survey and more cautious than a total theory. It offers a coherent model, explicit formal bridges, cross-scale case analyses and a falsifiable research programme. It does not claim to have solved the hard problem of consciousness, derived ethics from physics, or proven that living systems instantiate exceptional algebras. Its achievement is to relocate these uncertainties inside a common architecture in which their exact status, mutual constraints and empirical openings become visible.

Completing the Life-Value Onto-Axiology Program: From Life-Ground to Living Coherence

Life-Value Onto-Axiology (LVOA), developed by John McMurtry, offers a universal criterion for evaluating claims, practices, institutions, and economic systems according to whether they enable or disable life-capacities. Its central concepts — the life-ground, universal human life necessities, life capital, civil commons, the life sequence of value, and the money-sequence inversion — provide an unusually powerful basis for diagnosing contemporary social and ecological crises. Yet universal evaluative adequacy does not automatically provide a process ontology, causal model, measurement architecture, decision procedure, or domain-specific research method. This white paper proposes a minimal completion of that unfinished operational program.

The proposed reconstruction preserves McMurtry’s Primary Axiom while adding five connected layers: a process-relational account of living unities and objective need; an account of how significance, felt valence, and reflective meaning emerge in life-processes; a minimal grammar that prevents unmarked crossings among biological, experiential, institutional, and normative domains; a dynamic Need–Satisfier–Provision–Assimilation–Capacity–Margin–Viability model; and a threshold-sensitive decision procedure incorporating non-displacement, distribution, reversibility, and epistemic uncertainty. It further specifies life capital and civil commons as institutional forms for reproducing life goods and capacities through time.

The paper argues that LVOA can be considered conceptually complete only in an open sense: it now possesses the minimum constitutional architecture required to guide empirical inquiry and public action, while remaining corrigible by lived experience, causal evidence, adversarial testing, and affected communities. Applications are developed for health care, education, economics, and governance. The paper concludes with falsifiable middle-range hypotheses, safeguards against doctrinal closure, and a research program for canonicalization, instrument development, paradigm studies, comparative institutional testing, and plural governance of a Life-Knowledge Commons.
